While I am not contracted with any insurance companies, I will provide you with a billing statement that you can submit to your insurance company. 

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully.

A 24-hour notification is required for cancellation of therapy appointments.
